An affidavit is a legal document, in which the person signing it swears that the contents of the affidavit are true to the best of his or her knowledge and belief. The person signing the affidavit must have personal knowledge of the facts contained in it. The affidavit may be used for legal purposes such as obtaining a passport, visa or other documents from authorities.

Affidavits can be taken by any Notary Public in Canada or outside Canada and must be sworn before them. A Notary Public is an official appointed by provincial governments who has the authority to witness signatures on documents, certify documents and administer oaths.

Any Canadian citizen or resident of Canada over 18 years old may take an affidavit for use outside of Canada.

• birth certificates
• marriage certificates
• divorce decrees
• death certificates
• police certificates
• educational diplomas and transcripts
• military service records
• professional licenses (e.g., medical doctor, dentist, nurse)
• certificates of good conduct (police certificates)
• letters of recommendation from professors or employers (if applicable), etc
